You do not have to fill in every box. Required fields are marked with a red dot. Everything else is genuinely optional — but the more we know, the better the week works for everyone. Nothing you tick commits you to anything.

Your application is seen by the camp director and the planning group. Dietary and access needs go to the camp health centre. Certifications are recorded with their expiry dates so we know what is current. We use all of it to run the week safely, and for nothing else.

Your police record check is seen by one named person on the board and the camp director. We record that it was seen and cleared — not what it contained. To correct or remove your information, email info@canadianburnsurvivors.ca.
